Level 3 Mentor Evaluation
About Indiana Skill Assessment Scales
Indiana Skill Assessment Scales
The Skill Assessment Scales are an important part of Indiana's child welfare training program. Feedback is an essential way to help Trainees improve their helping skills. The process for the skills feedback is that the Field Mentor and Trainee have been completing the rating sheets at the end of each TOL period during the initial 9 week Training Program and weekly during the On The Job Training 3 week Training Period. These rating sheets need only address those skills observed/mentored during the specific review period.
This survey is different. This Skill Assessment Scale is to be completed at the end of the 12 weeks. It should be comprehensive and cover as many of the skills as possible. Please keep in mind that the purpose of the Skill Assessment Scale is to help improve training and skills. It is not expected that new trainees will perform every skill at a "superior" level. Objective evaluations help make the process work.
Later, the Trainee's Supervisor should complete a Final Skill Assessment scale three months following graduation based on the skills displayed by the new Family Case Manager during that time period. You will be notified when this is due.
Thank you for your efforts to help the children and families of Indiana.
